Investigation Summary
On April 22, 1992, Employee #1 was piloting a Cessna 172 on a part 135 FAA charter and attempting to land at the Shaw Island Airstrip. Employee #1 was attempting an uphill, and to the north, landing with an accompanying tail wind. Partway through the landing attempt, without having touched down, Employee #1 decided to do a go-around. The aircraft made contact with the tall trees surrounding the airfield and crashed. Employee #1 and his passenger were transferred to St. Joseph Hospital in Bellingham, WA. The next day Employee #1 died of closed head injuries. His passenger was released from the hospital and is back at work. The Shaw Island Airstrip is a difficult field to land at because it is 1,800 ft long, uphill to the north, and surrounded by tall trees. It is a one way in and one way out airfield and go-arounds are not permitted. In addition, there was an estimated tail wind of 10 to 15 knots. When the FAA and NTSB examined the aircraft, the flaps were extended 30 to 40 ft and the carburetor heat was on.
