Investigation Summary

At 9:13 p.m. on June 11, 1991, the winder crew was releasing pre-cut paper rolls. Once released, a roll hits strategically placed stops that break the roll up. Employee #1 was between the D and F rolls to prevent the F roll from falling. The D roll, which was approximately 56 1/2 in. high by 9 1/2 in. wide and weighed 700 lb, partially hit the stop and fell. It landed across the employee's lower legs and broke his right leg.
