At approximately 9:00 a.m. on September 8, 1993, Employee #1 was operating a winch that was anchored to an Ellis shore with blocked wheels. The shore gave way under the load being lifted and dragged Employee #1 into a hole approximately 22 feet deep. Employee #1 suffered bruises and abrasions over his entire body.
