Investigation Summary
At 5:30 p.m. on May 13, 1993, Employee #1 was operating a radial arm saw equipped with a dado blade used to make dado cuts in window bunk material. The dado blade amputated Employee #1's four fingers above the middle joint and lacerated his left thumb. The radial arm saw did not have a lower blade guard in place at the time of the accident.
