Event: 02/22/1991
Employee injured in fall while removing tank insulation

On February 22, 1991, Employee #1 and two coworkers were removing tanks and piping from the main factory area of the old U and I sugar beet factory north of Toppenish, WA. Employee #1 was assigned to strip insulation from an approximately 10 by 8 ft tank (#1) located about 10 ft off the floor. Another tank, #2, which is 10 ft in height and sits on the floor, was located 4 ft east of tank #1. Employee #1 was directed by management to place an approximately 1 1/2 in. by 4 1/2 in. by 12 ft board from the top of tank #2 to the top of a pipe located near the bottom of tank #1. Management then gave Employee #1 a chest harness with a rope, in place of a lanyard, tied to the D ring, and directed him to stand on the board and strip the insulation from tank #1. The employee stood on the board, tied the rope to an overhead catwalk grating, and proceeded to remove pipe insulation from tank #1. During the process, Employee #1 lost his balance, kicked the board out from under his feet, and fell. The rope untied from the catwalk, allowing the employee to fall 14 ft. Two coworkers found him when they went to get him for morning break. An emergency call was made to the Toppenish Fire Department. Responders entered the main factory to attend to the employee. He sustained back injuries and was hospitalized.
