Investigation Summary
On September 10, 1992, Employee #1 was trimming the wood with a chop saw. At about 4:10 p.m., the saw became stuck. Without turning the saw off, Employee #1 opened the safety gate and tried to reset the saw by pushing the reset bar close to the blade. His fingers were caught by the blade, and his thumb, index, and middle fingers were amputated.
