Investigation Summary

Investigation Nr: 14238661
Event: 11/04/1991
Employee injured in fall from bar joists to concrete floor

Employee #1 was working as a member of a team that was setting bar joists. When a crane lowered the bar joist into position, two employees would tack weld the joists into place and wait for the next one. As the crane was lowering the bar joist into position, the employees would guide the piece once it was close enough for them to grab it. As a joist came down, it suddenly started to free fall and fell about 5 or 6 feet. One employee, the connector, dove into a zoom boom basket close by. Employee #1 was either struck by the iron or lost his footing and fell approximately 22 feet 9 inches to the concrete floor. He was hospitalized. The crane operator had been flying in the steel or iron blind. He was aided by an iron worker talking him down with a radio, which is common practice. The crane operator was watching the load come down when he saw it suddenly free fall. He put the brakes on, causing the load to bounce and become entangled in the existing iron.
