Employee killed in fall from walk platform

Employee #1, an HVAC mechanic, was standing on the edge of an unguarded work platform approximately 12 ft 11 in. above a hardened floor surface. A coworker who was on an extension ladder handed an approximately 5 lb box of air conditioning filters to him. Employee #1 received the box, turned to place the box down, and apparently lost his balance. He fell backward out of the work platform opening, knocking open a wood swing-out door and falling to the concrete floor. Employee #1 died of a closed head injury the next day.
