Investigation Summary
Employee #1 was standing on a platform looking into the opening of a tall oil cook tank to determine if the liquid was circulating. Approximately 200 to 300 gallons of hot tall oil from the 5,000 gallon tank gushed out of the opening. Employee #1 was doused with hot tall oil. He received first-, second-, and third-degree burns over approximately 40 percent of his body.
