Employee injured in fall from roof

On or about 1:30 p.m., on October 25, 1995, Employee #1 was working on the roof of the Koury Convention Center. He was helping install a square-to-round piece of duct on the rolled angle flange of the joy axivane fan assembly. Employee #1 searched for a piece of plywood to lie on while he drilled two holes in the bottom of the flange. He saw a piece of plywood lying on the concrete roof, picked it up, and fell 18 ft through an 18 in. square hole in the floor. He landed on the next level, sustaining two broken wrists and bruised ribs.
