Employee's arm amputated in unguarded conveyor

At approximately 10:45 p.m. on July 21, 1995, Employee #1 was trying to reorient extruded rubber molding where it entered a hugger machine. The molding's guide rolls created a nip point. Employee #1 was wearing gloves that caught in the nip point, pulling his right arm into the in-running nip point of the parallel conveyors. His right arm was severed from the shoulder. The nip point at the conveyors had an adjustable guard, but the guard was open, exposing the nip to contact.
