Employee killed in fall to concrete floor

On October 19, 1990, Employee #1 was a member of a crew of six employees that was erecting a scaffold. Employee #1 was working at the 988 elevation when he stepped from a concrete floor onto a manufactured work platform that was hooked onto the scaffold. The platform moved and Employee #1 fell to a concrete floor at the 955 elevation. He died. One citation was issued for a serious violation of Part 12, R408.41217(8).
