Employee dies after fall from roof

On February 9, 1990, Employee #1, age 42, and five other employees of the Rochester Division of General Motors were on a roof relocating heavy duct work that had been lifted to the roof by a crane. The employees were sliding the duct on 4 by 8 sheets of plywood. Employee #1 was placing a sheet of plywood when it became stuck. When he backed up to reposition the plywood he tripped and fell through a roof opening that had been covered with black visqueen. Employee #1 died.
