EMPLOYEE'S HEAD CRUSHED BY FALLING FORKLIFT FORKS

EMPLOYEE #1 WAS ATTEMPTING TO ADJUST THE FORKS ON A CASE HI-LO. THE FORKS WERE RAISED APPROXIMATELY SEVEN FEET ABOVE THE GROUND. THE FORKS, WHICH HAD BEEN STICKING IN THE UP POSITION, SUDDENLY DROPPED TO THE GROUND. THE EMPLOYEE WAS KILLED WHEN HIS HEAD WAS CRUSHED BETWEEN THE FORKS AND THE GROUND.
