Employee killed when head crushed by rollers

On January 25, 1991, Employee #1, of Double Eagle Steel Coating, was completing the removal of an electroplating anode on cell #4 of a multi-cell galvanizing operation. The employee had not locked out the machine that was feeding stock through the cells prior to or during the manual removing of the anode. He was replacing the grating at the edge of the platform next to the two large in-running rolls. He slipped and his head went through the in-running rolls; he died. The employer was cited for several serious violations.
