EMPLOYEE #1 WAS WORKING IN THE CEILING AREA WHILE STANDING ON A 12 FOOT PORTABLE WOOD LADDER. ANOTHER EMPLOYEE AT GROUND LEVEL DID NOT NOTICE EMPLOYEE #1 AND ATTEMPTED TO MOVE THE LADDER OUT OF HIS WAY. THE LADDER MOVEMENT CAUSED EMPLOYEE #1 TO FALL. HE WAS KILLED WHEN HIS HEAD STRUCK THE CONCRETE FLOOR.
