Investigation Summary
Employee #1 and a coworker were preparing to bring a rail car to a repair yard. After inspecting the door and finding the east rollers off the rail, they barred the door onto the rail. Next, the employees attempted to close the door with Employee #1 positioned under the center of the door. The door guides above the door slipped out of a damaged area of the retainer and the door fell onto Employee #1. He sustained crushing injuries to his head and chest. He died.
