Investigation Summary
At approximately 1:18 a.m. on March 12, 1991, Employees #1 through #13 were bagging fluff in the distribution, maintenance, and shift departments of Building 245 when the oxide removal system in the ethylene oxide recovery tower exploded. The employees were injured by flying debris and from being thrown by the blast's force. Employee #1 sustained a blow to the head and died; Employee #2 sustained lacerations to the head; Employee #3 sustained a contusion to the right wrist; Employees #4, #8, #9, and #13 sustained multiple contusions; Employee #5 sustained contusions to the knee and head; Employee #6 sustained a laceration to the arm; Employee #7 suffered left knee pain; Employee #10 suffered multiple contusions on his left side; and Employee #11 and #12 suffered a contused back. They were treated at the hospital and all but Employee #2 were released.
