Investigation Summary
At approximately 11:30 a.m. on July 19, 1992, Employee #1 was told to go up on the roof of a warehouse and construct a roof of fiberglass panels topped by metal sections. The crew was working on Sunday because the company was behind schedule. Employee #1 and a coworker had just cut a fiberglass panel from a roll and dropped it into place on metal trusses. The coworker turned his back to Employee #1 and then heard him scream. Employee #1 either tripped or lost his balance and stepped on the edge of the fiberglass panel rather than on the metal panel. Employee #1 was killed when he broke through the panel and fell approximately 47 ft to the concrete floor.
