Employee killed when crushed by truck as jack slips

Employee #1 was assigned to replace the front axle on a 2 1/2 ton truck. He had removed the old front axle and had the replacement axle in position. The front wheels had been removed from the truck. Two jack stands had been placed under the springs on each side to support the truck while Employee #1 was on a creeper under the axle. One of the jack stands slipped, causing the truck to fall on the employee. He sustained a crushed chest and died.
