Investigation Summary
Two employees were dredging the bottom of a wastewater holding pond located on the LaRoche Chemical grounds at Gramercy, LA. Employee #1 was returning to shore from the dredge in a small aluminum boat and the other employee was on the bank edge waiting to receive the boat. A 1/4 in. cable used to position the dredge failed, causing the cable assembly--which was under tension--to suddenly snap back. It struck Employee #1 and broke his neck. It was learned that the manufacturer recommended 5/16 in. cable instead of the 1/4 in. that was used.
