Investigation Summary
At approximately 8 a.m. on July 17, 1991, Employee #1, the job foreman, was walking in the street to where the other employees were setting a 21 in. pipe in a 12 ft to 14 ft deep trench. Employee #2, the backhoe operator, was traveling in reverse, looking back to his right, preparing to turn into a driveway on his right. Employee #2 felt the right rear tire rise if as running over something soft. As he completed his turn into the driveway he saw Employee #1 lying on the pavement. Emergency medical services were summoned and Employee #1 was transported to the hospital. Employee #1 was dead on arrival at the hospital.
