Investigation Summary
Employee #1 was asked to place a grease fitting into a gear box cover so that the oiler could lubricate the gears. The gear box was part of a floor level conveyor used to move cars over the scale. The conveyor would cycle, moving about 20 ft, then stop for 5 seconds and then move another 20 ft. The employee failed to shut off and lock out the equipment prior to starting his task. He caught his arm in a pick-up dog and could not free himself. The conveyor moved and tore his arm off at the elbow.
