Employee killed in fall from apartment balcony

Employee #1, having just finished pressure cleaning a third floor concrete balcony, entered the building to go to the next apartment and to pressure clean that balcony. A coworker left the area to get cigarettes from his car. When he looked back, he didn't see Employee #1. He returned to the balcony, looked down, and saw Employee #1 lying on the first floor concrete patio. Employee #1 died. All witnesses, who arrived to help the fallen employee, unanimously agreed that there was no sign of the required safety belt.
