Employee dies in fall from elevated forklift fork

Employee #1 was putting a tarpaulin over a loaded flatbed truck. He was raised to the top of the stacked lumber by the forks of a forklift. Apparently, after he had spread the tarp, Employee #1 stepped off the truck onto the forks of the forklift and commenced pulling the tarp taut. For unknown reasons, Employee #1 stepped on only one fork. The driver states he noticed Employee #1 waver, start to lean, and then fall. Employee #1 was killed.
