Employee dies after fall from scissor lift

At approximately 7:45 a.m. on July 12, 1993, Employee #1 and a coworker were working from two separate scissor lifts. They were marking with a 25 ft tape where to place lights in the ceiling. Their lifts were raised to a height of 22 ft. Employee #1 did not have the chains hooked across his lift and he did not have a safety belt/lanyard attached to the lift. Employee #1 walked off the lift platform and fell to the concrete. He was taken to the hospital, where he died.
