Employee is killed in fall from roof

At approximately 10:50 a.m. on April 25, 1991, Employee #1 was working on top of a new construction roof placing sheet metal roof decking in place. When the first row was completed, he began work on the second row. As he was carrying a second sheet for row #2, the employee backed into an open area of the structure and fell 27 ft 4 in. to his death. A coworker, who was holding the other end of sheet roof decking at the time, said he yelled, "Stop!" three times to the employee, but he did not listen and continued to walk backward.
