Employee is struck by falling concrete and dies

On February 16, 1995, Employee #1 asked a forklift operator to use his forklift to move some palletized paper samples from the storage area to the loading dock. Employee #1 was on the loading dock directing the forklift operator through the opening between the storage area and the loading dock. As the operator drove through the opening, the backrest of the forklift struck the overhead concrete lintel, causing the lintel and the block wall to fall onto Employee #1. He died.
