Investigation Summary
Employee #1 was working in a 13 ft deep by 11 ft wide by 42 ft long trench installing materials for a drainage system when the side of the trench caved in and buried Employee #1. Employee #1 was extracted three hours after the accident because the trench continued to cave in while EMS attempted a rescue. Employee #1 was pronounced dead at the scene.
