Investigation Summary
At approximately 2:00 p.m. on October 9, 1994, Employee #1, a supervisor, told two employees that he needed to weld a 12 in. fabricated extension pipe onto an existing chilled water pipe. He was welding approximately 25 ft down in a 50 ft mechanical shaft. When Employee #1 completed the welding operation, he stepped off an 8 ft fiberglass ladder onto an I-beam and lost his balance. He fell and hit his head on the concrete floor. He died from his injuries.
