Employee killed in fall through roof opening

At approximately 1:45 p.m. on January 29, 1996, Employee #1 was working with a coworker on the roof of a building that was under construction. They were cutting an opening approximately 7 ft 2 in. wide by approximately 19 ft 6 in. long into the decking of the roof for an air conditioning unit. The ends had been cut and they were cutting the length on the east and west sides. Employee #1 had finished approximately 7 ft on the east side when he stopped and talked with the other employee. After the conversation, Employee #1 went to the north end of the section that had been cut away and tacked to prevent it from falling through onto the workers below. Employee #1 stepped on the section and fell approximately 17 ft to the concrete floor, striking a beam and a fire extinguishing line. He was killed.
