Employee killed in fall from roof

Employee #1 and a coworker were installing roof sheathing on a house with a 10-in-12 pitched roof. The coworker measured, cut, and handed the panels to Employee #1 by passing them up through the rafters from the second floor of the structure. Employee #1 had installed the row of panels along the leading edge of the roof and was beginning to install the second row of sheathing. He took a panel from the coworker and was about to place it when a gust of wind caught the panel. Employee #1 lost his balance and fell approximately 22 ft onto a concrete surface. He died of head injuries sustained in the fall. Employee #1 was not wearing any form of personal fall arrest system and no other means of fall protection was available.
