Employee died of asphyxiation

An employee in forklift operator training was assigned to operate (steer) a forklift (no power) in tow without direct supervision. The tow forklift was operated in a forward direction, and the forklift in tow was being towed backward. The employee started to drift left onto the shoulder of the road towards an embankment. The forklift tipped over causing the operator to jump. He was crushed between the over head rack and the ground. The employee was not wearing his seat belts. He died of asphyxiation.
