Investigation Summary
On February 16, 2002, Employee #1, a tank truck operator, was unloading liquid whey from the top of his tank trailer. He conducted the unloading operation without fall protection. As he attached a filler hose to the unloading nozzle located on the trailer's top, he lost his balance and fell. The height of his fall was approximately 11ft. He struck his head upon the ground causing fatal head injuries.
