Employee killed when struck by falling container

A 50 lb fiberglass container measuring 17 1/2 in. by 28 in. by 39 in. was being hoisted to the roof of a 5 story building to remove construction debris. It became unsecured and fell 5 stories to the sidewalk, striking Employee #1 on the head. He was killed.
