Employee killed in 37 ft fall through scaffold trap door

On or about February 19, 1993, Employee #1 was working from a scaffold platform that was attached to column forms. He removed a trap door (emergency door) to descend to the next scaffold platform and either slipped or fell through the door to the next level, then to the ground. Employee #1 was killed.
