Investigation Summary
Employees were removing the existing roof shingles of a two-story single family house for replacement. Employee #1 was assigned to carry the removed shingles and place them in a designated area from where they would be loaded into the bed of a truck that was parked at the side of the house. Employee #1 misjudged the roof edge, causing him to fall 17 ft 9 in. He died.
