Employee killed when crushed by falling lowboy trailer

Employee #1 was lowering a variable-height semi-trailer onto bolts that were to pass through the trailer bed and secure large upright transformers. When he let air out of the cylinders to lower the trailer onto a bolt, the bolt became jammed between the underside of the trailer's aluminum floor and the concrete floor. Employee #1 used a hammer that was under the trailer to knock the bolt into the hole without reinflating the air cylinders on the trailer. The bolt was knocked loose and the edge of the trailer fell approximately 4 inches onto the left side of Employee #1's head, crushing/compressing his skull and brain against the concrete floor. He was taken to the hospital and placed on life support, which was withdrawn the following day. Employee #1 died.
