EMPLOYEE'S FINGERS AMPUTATED BY DE-BONING MACHINE

Employee #1 was in the process repairing or replacing the shear pin on the main drive of a Food Craft de-boning machine. Another employee activated the machine. Employee #1's fingers were amputated. A Lockout-Tagout operation was not performed. Employee #1 was hospitalized for his injuries.
