Investigation Summary
At approximately 11:00 a.m. on September 19, 1995, Employee #1 was lying on the work platform, reaching under a six-station injection molding machine. The machine, which operated on a 32-second cycle, engaged and rotated, catching him between the platform and the bottom of the machine. He was pulled through two stations and his head was crushed between the bottom of the machine and the metal plate beneath the injection arm. Employee #1 was killed.
