Employee killed when struck and run over by front end loader

Employee #1 and the superintendent were getting ready to spread cement and sand, which had just been dumped by a front end loader, on a pothole. The loader operator was heading back to get another load when Employee #1 apparently walked in front of the loader. The bucket struck Employee #1 in the head, causing him to fall to the ground and roll to the right. The right front wheel caught his head and spun him around as the wheel ran over his head. He was pronounced dead at the scene.
