Investigation Summary
Before 12:00 noon on May 27, 1993, Employee #1 was seen on a chip barge. He then apparently attempted to cross from the barge to the mill dock by traversing the gap between the dock and barge, using a horizontally placed ladder. The ladder apparently slipped, causing Employee #1 to fall between the floating logs (camels) and the barge. He was probably crushed to death when waves closed the gap between the logs and the barge. Employee #1's body was found in the water at 2:25 p.m. No one witnessed the accident.
