Summary Nr: 79754.015 Event: 09/16/2015Employee Is Killed In Fall
At 10:30 a.m. on September 16, 2015, Employee #1 was working from an aerial lift , on an uneven surface, spraying an application of fire proofing materials to th e structural beams of a warehouse that was under construction. When an attempt w as made to move the aerial lift, the equipment's position relative to the inclin e of the work surface caused it to run up and off of one side of the uneven elev ation, and fall over. As it fell, it threw Employee#1 to the surface below, kill ing him.
