EMPLOYEE'S FOOT RUN OVER WHILE RIDING ON A FORK LIFT TRUCK

ON APRIL 6, 1992, EMPLOYEE #1, THE PRESIDENT OF THE COMPANY, WAS RIDING ON THE FORKS OF A FORK LIFT THAT WAS DRIVING TO AN AREA OF THE PLANT WHERE HE WAS GOING TO BE LIFTED TO A 2ND-TIER LEVEL 5 FEET 10 INCHES ABOVE THE FLOOR. EMPLOYEE #1 SLIPPED OFF THE FORKS AND HIS RIGHT FOOT WAS RUN OVER, RESULTING IN SEVERAL LACERATIONS AND A FRACTURE OF THE FOOT.
