Employee Killed In Fall From Unstable Platform

On May 25, 2003, Employee #1 was assigned to change a security light bulb, while standing on a platform that was placed on a fork lift. After changing one bulb, he shifted his weight to change another bulb. Suddenly, the platform slipped off the forks, and Employee #1 was thrown off the platform, falling approximately 18 feet to the concrete surface. Employee died at the scene.
