Employee dies in fall while installing roof deck

At approximately 9:30 a.m. on December 23, 1994, Employee #1 was 23 feet above a concrete floor, installing a roof deck. He was stapling insulation together in preparation for installing the next roof panel. According to employee statements, Employee #1 lost his balance and fell forward to the concrete, sustaining fatal head injuries.
