Investigation Summary

Investigation Nr: 14422109
Event: 12/20/1994
Employees burned in molten steel explosion

At approximately 12:30 p.m. on December 9, 1994, all steel casting molds had been filled, leaving about 2,000 pounds of molten steel in a ladle that was supported by an overhead crane. The ladle was moved by the crane, operated by Employee #1 in the overhead cab, to an open mold--the Pig Pot. Employee #2 was at ground level operating the valve that allowed the molten steel to drain into the Pig Pot. After a significant amount of steel had been added to the Pig Pot, an explosion occurred, which expelled molten steel in all directions. Employee #1 suffered burns on his face and hands. Employee #2 suffered third-degree burns on his legs, arms, and the right side of his face and body (approximately 60 percent of his body).
