Employee dies when struck by falling steel

At approximately 6:00 a.m. on August 17, 1994, Employee #1 was operating an upender machine that went by the stop position to the lower limit switch. Employee #1 went under the machine, opened up the reset control, and pushed the down switch, which then tilted the bed 28 degrees from horizontal and dumped a 5,000 lb coil of steel onto his head. Employee #1 died.
