Investigation Summary
AN EMPLOYEE WAS INSTALLING A MICROWAVE ANTENNA AT A RESIDENCE. THE EMPLOYEE PLACED A 28-FOOT EXTENSION LADDER, IN A RETRACTED POSITION, AGAINST THE HOUSE TO GAIN ACCESS TO THE ROOF. HE HAD PLACED THE LADDER AT A SHALLOW ANGLE ON A CONCRETE DRIVEWAY THAT WAS SPARSELY COVERED WITH FINE GRAVEL. WHEN THE EMPLOYEE COMPLETED THE INSTALLATION, HE TRIED TO GET BACK ON THE LADDER FROM THE ROOF WHILE HE HAD EQUIPMENT IN HIS HAND. THE BOTTOM OF THE LADDER SLID AWAY FROM THE HOUSE, CAUSING THE EMPLOYEE TO FALL IN A CARTWHEEL FASHION TO THE GROUND. THE EMPLOYEE STRUCK THE BACK OF HIS HEAD ON THE PAVEMENT. (THE ACCIDENT WAS WITNESSED BY THE HOMEOWNER.) THE EMPLOYEE WAS HOSPITALIZED FOR HIS INJURIES, BUT DIED 2 DAYS LATER.
