Employee's head crushed by a plant machine

While making adjustments on the consolidator Employee #1, a mechanic, was injured when a supervisor accidentally pushed a pneumatic air valve to the "on" position. This action caused the machine to operate trapping and crushing Employee's #1 head. Employee #1 was hospitalized for his injuries, and was undergoing rehabilitation.The consolidator had two power sources, electrical and pneumatic. The electrical power source was turned off and locked out by coworker, working on the circuit. However, Employee #1 working on the consolidator did not tagout the air line providing pressure to the machine. The valve was not capable of being locked out nor there were any markings on the valve to indicate the "on" and "off" positions. The company had no written lockout-tagout procedures for rendering the machine safe to work on.
